Proxies such as Squid will not support H.323 and as such this will not work.
If you are using a Linux box (as squid would suggest) then take a look at www.gnugk.org (http://www.gnugk.org) where there is a freely available gatekeeper and H.323 proxy which will allow your H.323 traffic to traverse your NAT.
Radvision, VCON and Cisco also manufacture Application Layer Gateways for H.323.
The only other way of doing this is to set up a 1:1 'outside to inside' NAT mapping. You cannot use PAT as the media streams are set up typically set up on dynamically assigned ports. The other requirement for this method is for your H.323 endpoint to support operation behind a NAT. In this way the endpoint will place the outside NATed IP address in the H.323 payload instead of the non-routable inside address.
GNUGK is probably the easiest route for you to start with, however help on setting it up is limited to a mailing list on the GNUGK web site.
Good luck.
